Apply For Assistance

You must be over 18 and a full-time, legal, permanent resident of Georgia. If the applicant is under 18 years of age, a parent or legal guardian must complete and submit the financial assistance application on their behalf.
You must be in active treatment (i.e. Chemotherapy, Radiation Therapy, Immunotherapy at a Georgia facility to be eligible for assistance.
Applicants can apply for maximum award of $500 per calendar year.
You must upload a signed copy of our oncology certification form. This form must be signed by your doctor and indicate that you are in active treatment at the time of your application. The form may be uploaded as a picture or a scanned document.

All applications are reviewed at scheduled board meetings. It may take up to 60 days to receive a notice of approval or denial regarding your application.

Our organization provides financial support based on available resources and eligibility criteria. Assistance is not guaranteed and is subject to change without notice.

Completing an application does not guarantee funds. Funds are dispersed based on need and funds available regardless of treating physician or where treatment is received. Utility bills must be provided and will be accepted in the name of a spouse, partner or caretaker living at the same address. Payments are made on their behalf to corresponding companies. We provide gift cards for gas and transportation (Uber/Lyft). The information provided on your application will be kept in the strictest confidence and used only for the purpose of determining eligibility.

Cancer Connections may communicate with your oncology office as needed to verify or clarify information in your application.
