5K RUN/WALK REGISTRATION

ONCE REGISTERED YOU ARE ACCEPTING THE WAIVER BELOW

We’d love to have you join us! To participate in our event, simply complete the Event Registration Form below and submit the $25 registration fee. Once your form and payment have been received, your registration will be confirmed automatically. By registering for this event, you acknowledge and accept the event waiver and release of liability.

  • There are no on-course restrooms. They are located by the concession stand.
  • Bib Packet Pick-Up will begin promptly at 9:00am
  • Race begins at 10:15am
  • No T-shirt race
  • The 5K course has a 1-hour time limit. The course will close once 1 hour has passed. The course time limit will begin when the last wave of runners and walkers crosses the start line.

PARTICIPANT WAIVER, RELEASE OF LIABILITY & MEDIA RELEASE

I further understand that I am solely responsible for my health and safety, and I acknowledge that I am physically and medically capable of participating in and completing this Event. I agree to abide by any decision of an event official regarding my ability to safely complete the Event. I further agree that Event officials or volunteers may authorize necessary emergency medical treatment for me.

I understand that it is my responsibility to know what personal equipment is required (such as appropriate footwear and clothing), to provide such equipment for my participation in the Event, and to ensure that it is in good and suitable condition. I agree to ask questions whenever necessary to ensure I understand how to participate safely.

Assumption of Risk

In consideration of being permitted to participate in the Event, I voluntarily acknowledge and assume all risks associated with participation, including but not limited to:

  • Falls
  • Broken bones
  • Strains and sprains
  • Heart attack or other medical emergencies
  • Property damage
  • Contact with other participants
  • Weather conditions, including extreme heat and/or humidity
  • Traffic
  • Roadway, trail, or track conditions

I understand that these risks are known and appreciated by me.

I also understand and agree to abide by the Event rule that no pets, rollerblades, or roller skates are permitted during the race.

Release of Liability

In consideration of being permitted to participate in the Event, I, on my own behalf and on behalf of my heirs, representatives, executors, administrators, and assigns, hereby release, relieve, covenant not to sue, and forever discharge, defend, indemnify, and hold harmless Cancer Connections INC, its Board of Directors, volunteers, sponsors, and agents from any and all claims, demands, rights, liabilities, losses, expenses, and causes of action of any kind or nature, including claims arising from negligence (but not gross negligence or willful misconduct).

This Release includes, but is not limited to, claims arising from known or unknown, foreseen or unforeseen bodily injury, personal injury, illness, property damage, catastrophic injury, or death resulting from or connected in any way with:

  • Participation in the Event
  • Activities associated with the Event
  • Travel to or from the Event
  • The Event, whether or not it actually occurs

Acknowledgment

I ACKNOWLEDGE THAT I HAVE CAREFULLY READ, FULLY UNDERSTAND, AND VOLUNTARILY AGREE TO THE TERMS OF THIS WAIVER, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK.

I intend this to be a complete and unconditional release of liability to the fullest extent permitted by law. If any provision of this Release is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

This Release shall be governed by and interpreted in accordance with the laws of the State of Georgia, without regard to its conflict of law principles.

Media Release

For good and valuable consideration, the receipt and sufficiency of which are acknowledged, I grant Cancer Connections INC, and those acting with its authority and permission, the irrevocable and unrestricted right to create, use, reuse, publish, and republish photographs, video recordings, audio recordings, or other media containing my likeness or voice (collectively, the “Recordings”), including those captured during the Event.

The Recordings may be used in connection with publications or materials supporting the mission of Cancer Connections INC, including but not limited to:

  • Advertisements
  • Brochures
  • Promotional materials
  • Social media
  • Websites
  • Printed publications
  • Any other media now known or later developed

The Recordings may be used with or without my name.

I acknowledge that Cancer Connections INC owns all rights, title, and interest in the Recordings, including all copyrights, and retains the unrestricted right to edit or modify them. I assign any ownership rights I may have in the Recordings to Cancer Connections INC.

I waive any right to inspect or approve the Recordings or any finished materials incorporating them, including materials containing my name. I further acknowledge that Cancer Connections INC is under no obligation to use any Recording.

Parent/Guardian Consent for Minors

If the participant is under eighteen (18) years of age at the time of registration, the participant’s parent or legal guardian must review this Waiver and Release in its entirety.

The parent or legal guardian acknowledges that they have read and understand this Waiver and Release, consent to its terms on behalf of themselves and the minor participant, and authorize the minor’s participation in the Event.
